If you’ve ordered from Applebee’s recently, you might have noticed an invitation to provide feedback on the bottom of your receipt. Don’t throw it out! Using your unique survey code, check number, server’s name, and the time of your visit, you can get a chance to win $1000 in the Daily Sweepstakes and $1500 in the Weekly Sweepstakes, just for providing feedback about your visit.

To enter, you need to be over 16 years old at the time of submission and a legal resident of the US, Canada or Puerto Rico. Only one entry per household will qualify each month, and you’ll need to complete the full survey for your entry to count.

The survey will only take around five to ten minutes to complete and consists of simple questions gauging the quality of your experience, food, and service during your visit. It’s definitely worth holding onto your receipt for!
